Fences and hedges behave like solid walls in that they stop the smoke grenade's cloud from expanding. This makes no sense, not to mention being inconvenient.
This didn't happen in DOS - I just retested right now, just to be sure I wasn't going crazy.
(I wonder how flares are affected? I never play night missions, aside from Cydonia, and Martians obviously haven't been growing hedges there. But I do wonder.)

Yeah flares are not effected that way.
Never noticed the difference, but that would be nice to fix.

that's just how that tile spawns. unless you're sure you threw an extra smoke grenade inside the door first.

I did.
And I expect when it's just the tile spawn, it's for the same reason. This is because exploded Terror Ships' Power Sources affect each other in a chain reaction, and that seems to indicate that pre-mission explosions obey the same engine physics as what happens during the actual mission. I'd be suprised if that cloud spawned like that in the original X-com.
But one way or the other, yes, I did throw a smoke grenade there.
