This mod changes Battle-scarred and Patient awards to give penalties to agent stats, not bonuses, in X-Com Files Mod.
Instead of receiving an increase of +1 health point per Battle-scarred award your agents and dogs will receive a reduction to their health. In addition the Patient awards will not give stun recovery bonus anymore, they will reduce time units and stamina.
The penalties will not end in negative stat values, I've checked this. If an untrained agent (https://xcf.trigramreactor.net/master/article/STR_SOLDIER) or dog (https://xcf.trigramreactor.net/master/article/STR_DOGE) with minimal starting health value of 25 will somehow achieve the tenth level of Battle-scarred award with 31 hp penalty the resulting health will equal 1, not -6.
Installation is simple: just unpack the attached archive into "/user/mods/" folder. In the end there must be at least two folders side by side there: XComFiles and BattleScarsHurt.
Enjoy the scars that hurt after hospital!

To start making this mod I've looked for strings "STR_MEDAL_PURPLE_HEART*" and "STR_MEDAL_CRIMSON_HEART*" in files "soldierBonuses_XCOMFILES.rul" and "commendations_XCOMFILES.rul", for strings "timesWoundedTotal" and "daysWoundedTotal" in savefiles. Below is a table with comparison of this mod effects with the unmodified X-Com Files. I've tested the mod on X-Com Files version 2.4 but I presume it will work on any version.

I don't want to rain on your parade, but... What's the point of this, exactly? Getting wounded is already pretty bad, now there's no upside at all and only downsides.
If you at least had some sort of 'chicks dig the scars' bonus tacked on somewhere, maybe. :-\

This mod is for those who want only downsides from wounds. To impose handicap on playthrough, for example, like there – X-Com Files Mod The Hardest by Fomka (Superman, Ironman, Handicaps) (https://openxcom.org/forum/index.php/topic,10581.0.html).

No thanks for explanation, just a labeling? How rude.
To make some sense out of it here another, not masochistic as you call it, foundation for the mod – real life. I just don't like the approach when someone was heavily injured, spent 20 days in clinic and then is able to run and shoot as usual (with even increased Health). Real fatal wounds are more serious than that.

It was tongue-in-cheek, check the smiley. Anybody who even tries to play through a significant part of XCF qualifies as a 'masochist' from a certain POV. Myself included.
Trying to live RL is the ultimate exercise in masochism. :P

Strangely to me the Battle-scarred award is given even if no fatal wound was received. It seems it is given for every unit that hasn't full health at the end of a mission. Even if the difference is only 1 hp.
Here is that insight from my playthrough
...Bolt the dog was scratched by the last cultist with a throwing knife. In spite of he receives no fatal wound, he is considered Battle-scarred and suffers from -1 hp awarded by Battle Scars That Hurt (https://openxcom.org/forum/index.php/topic,10596.0.html) submod. OK, I'll live with that
